How HCCs help hospitals 3M Health Care

One way we can do that is just by doing a chart review. So go into a facility and look at one patient across an entire year through clinics, ambulatory surgeries, ED visits, inpatient visits, to capture all the information and then determine how high the risk score is for that particular patient and look to see if there's opportunity / 4 / to improve that based on the hierarchy that I explained before so that you can get to the highest level within a disease category. So if there are opportunities, then those could be presented and we could translate that into how much you can improve your risk score for that population.
